Here are the Jump Start skydive waivers, medical clearance & RNZAF carriage forms for Jump Start .
All forms must be completed and returned to us no later than 20th April 2012.

1 – Jump Start Registration form- The following form must be completed by all teenager participants from CanTeen / Project K in full.

2 – RNZAF Tandem Screening form – we need this back from each Jump Start teenage participant in time to be checked by our Doctors on base two working weeks before the event date (by 20th April 2012) these will be treated as medical in confidence. This includes teenagers from both CanTeen & Project K.

3 – MD 440 & MD 440A- these are the forms that allow a civilians to fly on the RNZAF aircraft. One is for adults and one for minors. Anyone intending to fly on the RNZAF aircraft must complete one of these. (this applies to ANYONE scheduled on the Hercules -  all skydivers, tandem masters, camera flyers, tandem passengers, observers & media personnel )

4 – Participating Skydiver forms- If you are jumping the Herc as a participating skydiver, these two forms must be completed. (this applies to – all sponsoring skydivers, non-skydiver sponsors (Observers) and all Tandem Masters & Camera flyers in the working crew)
Please complete both of the following forms.

All forms need to be completed and returned in full to Jump Start no later than 20th April 2012.  Please do it TODAY.  Thank you.
